Answer
The application form for a new IATA Airline Code (designator or 3 numeric code) can be obtained at the following IATA webpage:
 http://www.iata.org/services/Pages/codes.aspx
 
Once the application form is completed it should be returned to IATA by logging a query through the IATA customer portal at www.iata.org/cs and attaching the completed application form. All other relevant documents for to meet the requirements should also be attached to the query (i.e. Air Operating Certificate, Certificate of Incorporation/Registry, Confirmation of Schedules and any other requirements that may be needed) Once all requirements have been met, IATA will send a Pre-payment Advice for the collection of fees. Please do not send any payment before receiving the pre-payment Advice.
